I would recommend implementing the SQL standard behaviour (UNIWUQE constraint
on nullable columns) as a first step.
A second separate issue could be to add additional syntax to CREATE INDEX, but
I personally don't see the point. Why add new alternate syntax that will
confuse the user (just have one way to achieve one thing), especially as the
new syntax will be not standard and a standard way exists.
