no extension can do that ("Extension" in the sense of not touching OOo
source code in order to make your extension work -- in this "strict"
sense, the SRB is not an extension)
So "Do I need anything else?", yes: learn C++, hack OOo, and make your
own build :-(
A dummy thing, like just hacking
dbaccess/source/ui/app/AppIconControl.cxx by adding a new TPair to the
pTypes in OApplicationIconControl::OApplicationIconControl (not a
functionality, just an icon), would take you a lot of work (I'm just
guessing, 'cause I've never done it -- I'm looking forward for my
southern-hemisphere-summer-of-code)
Nevertheless, extensions can use *own* tool and menu bars, and can
*merge* their own in OOo's.
