I agree. If people don't have (or take) the time to do the necessary work, the results aren't very good. The OOo Setup Guide (in English) is a good example: the last time I looked at it, the page breaks and other layout features were just awful. It looked to me like an early draft, not a published document.
A last resort would always be to produce ODF, and export to PDF from there after fine-tuning the layout.
That is the only way I can see to get what I want, but I would be really happy if acceptable PDF output could be created directly.
But we could think of a way of keeping index entries in the wiki. An easy way would be to use spans around index terms. There is a way, it's just software
