I've used YA (Yet Another) Helpdesk from the Macromedia Devex which is
fairly basic, but does the job. It was quick and easy to set up, but we
customised the code a bit to save users having to reenter all their details,
etc. It doesn't run very fast on our elderly dev server, but it is adequate
for our purposes.
Oh yeah and it was free.
