Paul,
1. Hosts can supply alternative to cffile or a sandbox
2. You'll probably have to pay a fair amount for SQL2K, hosts will create DSN or you will have a control panel to do so
3. You could do this pretty easily, just create a dummy file to get the current directory

I have an application.cfm which sets the DSN, the directory paths for images and a few other bits and bobs. Who will make sure these point at the correct directories on the server?
I have never had to host outside of my own servers at work before so any help on this bit would be appreciated.
