There is also the Abandoned Modules wiki page to move all wiki content to:
https://wiki.openmrs.org/display/docs/Abandoned+Modules

I think you should also put the readme in the root of it as Rafal suggests
telling users where the updated version can be found.

I couldn't find a wiki page detailing a "module sunsetting" procedure.  Can
you start one?
