[~schor], I have attached a filled-in template. Since I do not have a member or
officer status in the ASF, can you please check the template? If I am
misunderstanding this, I can also proceed, although I don't know where to
commit it (http://svn.apache.org/repos/asf/incubator/ doesn't seem to be the
right place).
