The easiest I can think of is to include a %post script in the spec file 
that simply checks if files exist in the appropriate templates-custom 
directory(s) and echoes a message:

*WARNING*
Templates-custom fragments were found in $directory. Please review these 
custom fragments before expanding the template and restarting the service.

Now, what would the script look like.... any volunteers?

Any thoughts on making a dynamic script that somehow could discover what 
templates directories the rpm installed to, then check the associated 
templates-custom directories, then echo the message above.
