Based on this paragraph - this should never happen? Or am I missing
something?
Currently, and by ICANN requirements, it is the gaining registrar who is
tasked with ensuring the validity of a transfer request. Most registrars
(including OpenSRS) email the administrative contact of the domain that is
coming their way, and the registry notifies the losing registrar only once
the administrative contact acknowledges the request.
The losing registrar has 5 days to either acknowledge or reject the request,

and if the request remains unanswered after five days, the transfer is

COMPLETED AND THE DOMAIN IS MOVED TO THE NEW REGISTRAR.

We have just had some domains time out awaiting register.com and
domainregistry.com response
