I saw this behaviour once. I went and had a coffee, started a new payment, and it went through. The bank listed only one transaction (the second one), and the explanation I assumed was that Tucows' payment processing system was temporarily out of order the first time 'round. If you can't wait, and resolving this immediately is worth the risk of having to pay for support if it turns out to be your fault, then call the OpenSRS after hours support line. Or wait another three hours, and phone 'em.
