it's a core problem for many companies - medium to large agencies tend to suffer from this the most (not always but often enough). it's because there's no process or the inability to adopt one.
usually, it's cultural. oh the horror stories i can tell! for ex: should it take 1.5 yrs to do a corporate redesign despite having 85% of the interactive resources available? i think not.
