My issue with the form is its misrepresentation to the user. At first glance you decide or estimate the time investment to complete the screen. Slight modifications or an increase is expected, so I would not be too put off by a few extra fields to fill out. What is unacceptable in my eyes, is the length and lack of feedback. It seemed an unending process, similar to the form version of matryoshka dolls.
