Thanks to Brooke Baldwin for pointing out that my initial request was a little unclear.
What I'm looking for are examples of large online forms (or sets of online forms that make up a larger data collection process). What is of particular interest to me would be examples where organizations have taken larger multi-page forms that exist in the "paper world" and converted them to some sort of online representation. What I'm hoping to gather here is not related to any specific project I'm working on. Just looking to put together a reference for members of the company to think about as we progress in moving many of our paper forms and applications to a web environment.
