Which particular form are you referring to? They seem to have several
online forms at different countries. Is it one of these?

I recall seeing an article describing a usability review about that
precise site donation's form. I think it was this one:

... which mentions an increase in donations after the suggested design
was rolled out. Maybe the collection of links in that design (the
"Ways to Donate") is not that bad?   Which are the points that you
find pretty weak?
