I consider where I work to be a smaller shop <100 servers and I prefer Zabbix for my alerting. I have it page to the appropriate people on appropriate things. To clarify, I will have it SMS the primary admin for a server being down or a critical service that is down, but not if the server has a high cpu for 20 minutes. For the lower priority items a simple email will do and they can look at it when they get in.
I am not in favor of getting a SMS or even an email for every event because of desensitizing the person getting them. With any monitoring solution the biggest hurdle is eliminating the false alerts but still catching the things that are important. If you send alerts for everything then the person getting the alert quickly starts ignoring them because they assume it is just another false alert. One of the things I like about Zabbix is that it has escalation capabilities, so it will keep bugging you until the alert is cleared and if it doesn't clear within a set time it will alert the next person on the list. This is just my opinion though.
