Hobbit (now "xymon") also gets my vote. The biggest reason is, in my opinion, the simplicity of the interface. I got turned off of other monitoring suites because the interface was.. 'busy'. Xymon, like all of the other tools mentioned in this thread, meet your requirements.
Anyway, xymon works well for small sites, has the features you want, but also has scaled very well - I currently monitor about 250 servers thus far. No scaling issues that I've seen. The bottom line is, you just have to grab the free version(s) of the tools recommended here (there are SEVERAL as you've seen..), and just try them out. If nothing else, go to the source sites and at least check out the screenshots and see what feels right.
