Finding it difficult to repeat. Did just get the symptons by rebooting PC whilst playing, with that setting on 'pause'. When I changed it to 'stop' it oddly seems to corrupt the original now playing list (ie just 4 tracks from the album showing). In any case I have the feeling that this happens more often than I reboot.
Think I need to keep an eye on it so I can describe a repeatable sequence that gives the behaviour. You'd think I know after a year of living with this.
