It may be coincidence but another BBciPlayer user has posted a message about getting the "Error- Item not found in playlist." message on a local radio stream.
I have seen this message happen with some security software which buffered up http responses to examine them but some chunks of http were left in the security s/w buffers presumably waiting for another read.
