SSH Autotunnel won't do, as it can only forward one port, but you need to ports (9000 + 3483). If you want to use some maintained and official program from the store, ConnectBot is the way to go (as it supports multiple port forwards). When you use SSH tunneling "localhost" is the correct server address.
