Richard,

Open applications, open utilities, open disk utility, interact with the drive table and choose the drive you wish to perform actions on. The default tab will be first aid and this is where repair and modify permissions is found. Press the space bar on repair permissions and then you can use VO keys to check the progress.

You won't have to reboot after the first verification and repair but you'll have to reboot after updating. Don't forget: it's not enough to just repair permissions, you have to verify and then repair. I personally noticed VoiceOver running more smoothly after the process.
