Voice over was most definitely tested by users.  I met several of  
them when I was at the acb convention in Alabama in 2004.  They all  
had issues, though I don't recall anything about mouse keys coming up  
in conversations I'd had with them at the time.
There are a couple here on this list, but if they choose not to speak  
up, that's their choice.  But, I can assure you Apple did test this  
with users, and did not keep testing in house.
