The item in the side bar is mysql gui tools for 5.0 over here and the package has no preference pane and no readme file in it. I think you got a better package than what I managed to find.

The DMG contains four items, a Read Me file, a Preference Pane, and two packages: mysql - 5.0.45 and mysqlstartupitem.

I am able to touch all four items with VoiceOver and launch the installers selecting and launching with a variety of methods.

In a finder window within the Sidebar do you have an item named: mysql-5.0.45-osx, etc etc?
