I cannot reproduce this problem. It works just fine for me. See attached sample files.

Please make sure

1) to set the "type" attribute of element <note> to "trouble" (not "troubleshooting");

2) to pass XSL Utility parameter use-note-icon="yes".

I've not tested this with XSL Utility 5.4.5 (latest version is 5.4.6) but I think that, out of the box, it should work too.



If you did (1) and (2) and it does not work for you, then this issue may be caused by your XSL stylesheets customizations. (Just trying to guess.)
