Found the problem.

I had values specified in the note-icon-list parameter. These values were 
specified long before troubleshooting notes were available. So, I added 
"trouble" to the list and all is well.

using XSL Utility 5.4.6. See attached screenshot.

Just guessing:

- Do you have an icon when you generate PDF? (Please don't forget to set 
use-note-icon="yes" for this conversion too.)

- Just in case, please retry after

1) Making a backup of your user preferences directory.

%APPDATA%\XMLmind\FOConverter\

Example: C:\Users\julie\AppData\Roaming\XMLmind\FOConverter\.

2) Deleting your user preferences directory to restore the "factory settings".

3) Restarting XSL Utility.

4) This also means that you'll have to specify use-note-icon="yes" again after 
restarting XSL Utility because by default, note icons are not generated.
