I did (1) and (2) exactly as you did. I have not customized any stylesheets, but I do have a file that overrides some of the formatting -- nothing that I can see that would affect the trouble note without also affecting the other note types. I use tips and important notes on a regular basis, and they format properly. It's just the trouble note that is not formatting properly. I'm really confused as to what could be causing this problem.

See attached sample files. Please make sure 1) to set the "type" attribute of element <note> to "trouble" (not "troubleshooting"); 2) to pass XSL Utility parameter use-note-icon="yes". I've not tested this with XSL Utility 5.4.5 (latest version is 5.4.6) but I think that, out of the box, it should work too. If you did (1) and (2) and it does not work for you, then this issue may be caused by your XSL stylesheets customizations. (Just trying to guess.)
