DEFAULT_ITEM_READ and DEFAULT_BITSTREAM_READ can only apply to a collection.

So if you go to an item which should be restricted and do Edit ->
Authorizations
It shows

Item Policies
Action READ
Group YourGroup

and the same for
Policies for the Bundles and Bitstreams

There are no additional policies for Anonymous?

The group which should habe access is not e.g. populated by IP based
authentication or something similar?

At time of creating the collection, the group had been populated with
members, but currently one group is empty. I have added people back in, but
that does not seem to change anything.

I have use Advanced policy manager at some time, but can't recall if I also
used it for this particular collection.
There are no items in this collection under embargo or for which embargo
has been lifted.

How did you restrict the access?
There are two setting in a Collection
DEFAULT_ITEM_READ and DEFAULT_BITSTREAM_READ
  From these an item's default resource policies are derived.
Setting DEFAULT_ITEM_READ to other than anonymous will result in both
the item and the bitstream being accessible to the group set.
Settting DEFAULT_BITSTREAM_READ will leave the landing page accessible
and only restrict the bistreams.

These settings work only during ingest, changing them will not affect
existing items.

Did you at any time use the advanced policy manager?

There is one scenario where policies are not properly set see
