Right after I posted the below, I found this Microsoft knowledgebase article
http://support.microsoft.com/kb/917607 About halfway down the page there is a section "How to enable macros on a single computer after you install the WinHlp32.exe download." I clicked the "Fix It" button in this section, followed the steps, and now my problem is gone. The "old" DXbase help system works OK now under Windows 7 32 bit.
