Was the nut on the headphone jack tightened sufficiently to make the washer behind the panel tight? If not, there is a possibility that the board is moving a bit when the headphones are plugged in (or removed) and may cause "strange happenings". Squeeze the board and front panel metal together and snug up the headphone jack nut to strong finger tightness.
