I don't understand a 'several month lead-time'. When I was producing copy for manuals, we had the proofs in a couple of weeks, and that was when computer-aided publishing was a new thing. Besides, if it's a big and complex manual, you can leave a space on page 299 to drop the DoC in at a late stage. Even Caxton could have done that.
