Ghery, Or, as happened to Comair Flight 5191 out of Bluegrass Field (Lexington, Kentucky) on August 27, 2006, got confused taxiing out from the terminal for a pre-dawn takeoff, and *tried* to take off from the wrong runway. They didn't make it... The first officer was the only survivor out of 50 people on board.
