Ground/earth continuity is extremely important for electrical medical equipment, at least in part because sedated or comatose patients can lack a "let-go" reflex. Clear plugs presumably allow users to periodically visually check the robustness of the conductor/pin connection.

When moulded plugs were first introduced there was a serious reliability problem, with melting, sparks and smoke. Moulding techniques had to be considerably refined.
