You would change the user name, but otherwise use the same ...:extssh:... line you found earlier. Also, you can export the variable CVS_RSH=ssh in your .bash_profile, and it may work with only ...:ext:...
The change to an ssh-based login also affected my ability to do checkouts. I normally work on a Windows PC (I know I know - don't say anything) where I use TortoiseCVS. This can be set up to work with ssh, but I just haven't had the time to do it yet. (I know you can use puTTY and friends to manage ssh sessions and keys, but haven't gotten to the details).
