This is some of what I've found.

If you have a horizontal mill then the table is set to the gear angle.  The X 
drive and indexer are geared together and as the X moves the indexer turns.

The G3617 and the now G0757 can be mechanically configured to do this.  I 
suspect the LinuxCNC code to do this would be simple?

But with a vertical mill that tilts the head and can't pivot the table it's 
also possible as this short video shows.  Again synchronizing is simple?

And with a single point gear using LinuxCNC.  This looks to be the most 
complicated.

Most of them don't show the complete set up from further back.  But they either 
till the indexer or tilt the head.
