Unread Messages Custom View

In what circumstances is the Unread Messages Custom View updated? Perhaps
naively, I expected messages there to disappear when read. I know I can
"Refresh Message List" or switch on "Unread Only" but it feels wrong that
that should be necessary. Am I missing something?
