I have had this happen with IE as well - variations on a theme, you can also
end up with a Question mark in the Dock, or entirely losing the app from the
Dock. I've also had IE show in the Dock as though it was running (arrow on
icon), but it wasn't. Trying to repeatedly relaunch the app resulted in the
Mac freezing up (mouse stopped moving) requiring a reboot.

Maybe that's what it is - it's 'sympathy instability'
