I think that command-return was a bad choice for the Send command, for obvious reasons. The return key is used so often; the double carriage return issue mentioned above; the fact that sending a message accidentally can be embarassing, etc. I would have chosen a key combination less likely to be hit in error, maybe a three-key combo. Is there any way for a user to change the keyboard command mappings in Entourage? For classic apps built with proper resource forks, we can do that in ResEdit.
