The ST-E2 appears to be only slightly more expensive than the cord
combination, but while the cord combination allows you to place the
550EX anywhere the cord will reach, I am concerned that the ST-E2
will require that the 550EX is within 40 degrees of the camera's
straight-ahead line. Would any user of the ST-E2 confirm that when
it is mounted on the camera hot shoe, the IR beam emitter is fixed in
position pointing forward? Can the ST-E2 be rotated horizontally
(like the flash head of a 550EX) so that the beam emitter points off
to the side or back of the camera?
I am used to my off-camera flash being _behind_ the lens....

You already receive some good advice. You can swivel the 550EX tso the
front faces the ST-E2 while the flash points at your subject. The ST-E2 IR
beam points forward only, no rotation like the flash. E-TTL does work with
the ST-E2.
What are you planning to use it for? Indoors? Outdoors? Macro shots?
