As luck would have it I received my Arthur Morris book about bird photography 
today from Amazon.  Just leafing through it but not reading it in detail yet, 
I see that he addresses this exact issue.  He recommends pressing down on the 
lens directly over the tripod mount with your hand to dampen vibration and 
also not using a cable release, but, rather, using the camera against your 
cheek and pressing the shuitter manually as a point of stabilization.
