I see that advantage. That would be quite an exciting lens. The only
thing to keep in mind is that DO does not seem to decrease the diameter
of the front lens. Somebody correct me here if I am wrong.

Hm, 70-200 is the most popular focal range among the tele zooms.
Therefore, I don't think that this kind of lens is mostly used for
sports and action photography. In contrast, I think such a lens will be
more often used for NOT FAST moving objects. For all these cases I
believe IS is VERY useful (if you don't bring a tripod with you which
many don't/can't).

In contrast to the 70-200/2.8 I believe the 300/2.8 or even the 400/2.8
is percentage wise more often used for sports and action photgraphy.
Further, it seems to me that among the photographers using these lenses
IS is considered an (big) advantage. Considering the price of these
lenses most photographers use these lenses professionally (except for
the few people who have too much money. Wish I would be one of
them...).

The 100-400 is sold for about $1500. The 70-200/2.8 IS will probably be
sold for about $1600 initially + $300 for the converter. That gives you
a focal length of 70-400 with max 2.8 (from 70-200) compared to max 4.5
at 100mm. I think this is worth the +$400. Even more so because the
price will decrease within a year or so, because you (and many others)
believe that fast lenses are worth a bit more, because the range of
70-100 can be important, i.e. for portrait, etc. And then the 100-400
is a push-zoom etc. etc. I still would go for the
70-200/2.8IS+converter although it's ~$400 more then the
100-400/4.5-5.6.
