I believe what you're looking for was available from KJP/Calumet.  It's
listed in my '99 catalogue as a Manfrotto MN302 Quick Time VR kit, 35 MF CC.
Some title!  It features "double sliding plate for 'Nodal Point'
adjustment", "Spirit level", "Scaled Panoramic head" etc. and cost 199 GBP.
Try www.kjp.co.uk and also the Calumet or Manfrotto website.
