I would probably take the 28-135 IS and the 50 1.8, and leave the rest at
home--but that's just me.  I haven't taken a flash on my last 2 overseas
trips (Italy and Japan), and I haven't missed it at all.  Then again, both
trips were with only one other person so I was more focused on photographing
scenes rather than people--I imagine your objectives might be different with
your kids along.  :)

The 20-40 seems a little redundant, and I love to pack light so I'd leave
it, but I can see how it'd be useful in some of the tighter streets.  The
70-200/4 might be nice, but you probably wouldn't use it much in Rome, and
it's pretty conspicuous.  And if you bring the 28-135, you likely won't need
the 100/2.8.

One lens I found really useful in crowded Japan was the Sigma 28/1.8 (the
old one).  It's light, fast, sharp, and cheap--what more could you ask for?
