I think what you have to do is give up on getting both the background and
subject in proper exposure.  Instead, you'll have to accept properly
exposing the subject and underexposing the background.  I set my camera to
Manual Exposure Mode in these situations, still using E-TTL flash, at
400ISO, f/5.6, 1/30th.  Assuming you're flash metering correctly, then
you'll get a well exposed image for the background and an underexposed
background.

Many times I too don't want the backgrounds to be underexposed, or at least
badly underexposed.  In those situations, and if it's just a single person
portrait style situation, I'll drop the aperture down to f/5.0 or f/4.0 or
even lower, to pick up some exposure for the backgrounds.  The key here is
to carefully focus on the eyes.

Another technique to gain background exposure is to further increase ISO.
For a number of situations, I'm willing to go as high as 800ISO.

I have found that hand holding, at 1/15th or slower, does not generally lend
itself to a good image, even with an 'IS capable' lens, as subject movement
doesn't make the image look good.  I generally go no lower than 1/30th, and
thus try to pick up extra stops via aperture and/or ISO.
