I know of one. It was caused by loose or missing hardware, I recall on the passenger's side forward corner. I can't recall for sure, but the canopy may have been closed at the time of the incident. It caused some minor damage to the other attach points on the fuselage when it separated. It did not hit any part of the airframe when it departed. The most extensive damage was to the owner's wallet. No one on the ground was injured, AFAIK, and the canopy was not recovered.

(I'm not trying to make light of it, only to state the facts as I understand them)
