If people worked to that standard during the "Space Race"....pathetic. Made even more so when one considers that "documentation" is required these days in order that the right hand knoweth what the left hand doth...remember the Mars probes and the confusion over metric and US units. Are we seeing a pattern here? I think so.

We were looking at that yesterday, shock and disbelief growing with
each new revelation.  My verdict: "It's a shift change problem.  We've
