There were some embarrassing episodes during the Space Race too; they just
mostly didn't get as much attention as this one did. One gaffe after
another on the Ranger program almost killed JPL.

The "documentation" is mostly part of the problem, not part of the
solution. Its presence, in overwhelming profusion, should lead you to
expect more of this, not less.

Beware of seeing patterns where there may be only random chance. The
two cases are very different.
