If this is professionally installed, it would never have passed if it
had the wrong wire gauge. And installers don't like to put their name on
the line and will insist in using the proper gauge, so it must have had
the correct gauge from the beginning.
Since it was a new install (replacement charger) I wonder what the
reason was to replace the previous one - was it acting up? Was there a
problem with the wiring (interrupted or not properly connected/tightened
down)?
Especially if you use a dryer outlet for the 240V source, that could
have been an old outlet and if it was corroded, no lonnger able to
supply much current, so that could have been one (common) cause of
failure.
I have had a situation where a wire had been twisted back and forth
enough times to break enough strands internally that it could no longer
handle the current and eventually if cleanly burned off, leaving a lot
of soot and a
1/2 inch opening in the wire. No fire, luckily. And this was only 16A.
(No EV, it was a washing machine with an electric heater - common in
Europe)

I take that back.

I assume from the pictures that the charger they replaced was a wall
charger not in the car.

They most likely did not change to a larger gauge wire in the wall.
They probably just put in a different breaker.

The new charger pulled more current than the wire could handle and
burned up.

Be nice if they followed up with an explanation but we will probably
never know.
