Postfix plus Spamassassin on Suse, no complaints from the users (and I'm
sure they would if they weren't happy!) and on the admin side, to be honest
there isn't any other than the very occasional adding to/from manual
blacklists and whitelists.

If the "roll your own" isn't an option, take a look at Barracuda Networks,
never used one but they seem well regarded.
