Believe it, dude. The scenario I described is one I experienced first-hand not once but twice. Management spends a gazillion dollars on a SAN infrastructure, and because they have to justify their sunk cost, they tell you that you have to install Exchange on it, and there is no more money for additioanl shelves or drives, so you're just going to have to make do.
Of course, I would have designed it differently as well, but sometimes we have to do what the customer demands regardless of whether or not it will work right. (With plenty of warnings and disclaimers of course!) Or we're brought in after someone else's design, perhaps a management-forced design, has failed. I'm not sniping. I'm sharing. For years I've had to work with customers who've drunk the SAN Kool-Aid and from SAN salesmen who tell customers anything to make a sale [1], and I've heard from many more in forums like this who have a screwed-up infrastructure because they've been told stuff like "the cache will take care of everything." You could possibly put 160 mailboxes on a single SATA drive get acceptable performance. The rules are definitely different when you're hosting thousands of mailboxes.
