Do you have a firewall? You would need to open up various ports on it to allow direct access to Exchange from another network. The ports required for the various MAPI/NetBIOS/etc. operations (from Outlook) are different from those required for SMTP (which is server-to-server and has nothing to do with client access) or OWA (which is vanilla http).
Having said all that, it would be a Very Bad Thing to open up those ports. Do not do it. It would invite the entire Internet into your NT domain.
