Matrox Graphics, Inc. MGA 2064W [Millennium] (rev 1). The PC is an AMD K63/450. I'm actually pretty happy with this antiquated card, however. The fb console is accelerated and does 1280x1024 and is actually faster than several of the accelerated ATI R128 machines on my network. It's obviously not for games but does a great job otherwise.
